How to Use Disability Insurance for Maternity Leave
- 1). Review your company's leave policies. Some companies require that you use all of your paid time off, including sick and vacation days, prior to submitting a claim for disability coverage. You will find this information in your employee benefits handbook, open enrollment materials, company Intranet or in your summary plan description. If you need clarification, contact your Human Resource department.
- 2). Obtain the appropriate insurance claims forms from the disability carrier before giving birth, if possible. You should be able to download a form from the carrier's website. If not, you may have to call the insurance company and ask a rep to send you the form for submission. Your human resources office may have some forms on hand as well.
- 3). Fill out the disability claim form completely. If you miss any details, the insurer will return the form to you and you will have a delay in claim payment. Insurers also require your employer to complete a claim form. Notify your employer about your plans to go on disability leave and as soon as you actually do go on leave.
- 4). Ask your attending physician to complete her portion of the claim form or any associated paperwork required to process your claim. The doctor may also need to provide proof of disability. Make sure your doctor has signed off on the form and has given it back to you or the insurer.
- 5). Submit your claim form and supporting documentation, per the instructions on the form. Your employer may submit these forms on your behalf if it has all of the necessary documentation.
