We report an unusual case of cutaneous nephrocolonic fistula caused by a renal calculus with perirenal infection. The diagnosis was made by fistulography and computed tomography, after which nephrectomy and resection of the descending colon were successful. We also review the literature on cutaneous nephrocolonic fistulas.

Formation of spontaneous communication between the kidney and skin is a rare complication of calculi with acute or chronic infection. Formerly, primary nephrocutaneous fistulas were most commonly due to renal tuberculosis. Today, perinephric abscesses due to calculi are the most common cause of primary fistulas. Renocolic cutaneous fistulization occurs even less frequently. Only a few cases of cutaneous nephrocolonic fistulas have been reported. The incidence has greatly fallen in recent decades because of advances in diagnosis and treatment of renal diseases such as tuberculosis.

We report the case of a patient with persisting calculous pyelonephritis. Subsequently a perinephric abscess developed. A cutaneous nephrocolonic fistula was diagnosed by the combination of fistulography and computed tomography (CT).
