    • 1). Inspect the outside of the well cover. Look for signs of damage and wear. Covers should be free of dents and firmly set in the ground surrounding the well. Check for the presence of a lock on hinged covers that secure with a separate lock. Flush-mounted covers should have all of the bolts in place.

    • 2). View the interior of the well cover. The interior annular space should have either grout, bentonite, or sand partially filling the void to keep the cover firmly in place. The interior of the cover should also be free of dents, rust and provide sufficient space between the interior of the well and the well casing.

    • 3). Document the integrity of the interior and exterior of the well in a field notebook so that there is a record of the inspections. Note any damage that warrants repairs and when the repairs occur. Maintain these records until the well is properly abandoned.
