How to Apply for Social Security Disability in North Carolina
- 1). Review the Social Security disability qualifications on the Social Security Online website or go through the Social Security Administration's Benefit Eligibility Screening Tool online to confirm eligibility (see Resources).
- 2). Call the Social Security Administration at 1-800-772-1213 to schedule an appointment at your regional office in North Carolina. You also can fill out the application online on the Social Security Online website (see Resources).
- 3). Gather all of the items listed in "Things You'll Need" and place them into the file folder for easy access. Bring this file and all its contents to your appointment. If you are applying online, keep the file and its contents with you to complete the application properly.
- 4). Fill out and submit the Social Security Benefit Application with the information from your gathered personal, medical and work history information. Applying can be performed either in person at your scheduled appointment at your local North Carolina office or online at the Social Security Online website (see Resources).
- 5). Complete and submit the Social Security Disability Report using the information from your medical files. Again, complete the report either in person at your scheduled appointment at the local Social Security Administration Office or online at the Social Security Online website (see Resources).
- 6). Fill out the "Authorization to Disclose Information to the Social Security Administration (SSA)" form. You can download the form from the Social Security Online website and print it off from your computer (see Resources). This form can be submitted one of two ways: either present in person at your appointment or you can mail it to your local Social Security Administration Office.
